New fertility research reveals comparable embryo transfer outcomes for transgender men and cisgender women

Transgender men achieve similar IVF live birth rates and embryo outcomes as cisgender women, providing evidence for equitable fertility guidance. 

SAN ANTONIO, TX [October 28, 2025] – A new study from US Fertility highlights that transgender men undergoing in vitro fertilization (IVF) experience live birth and embryo development outcomes comparable to cisgender women who had a transgender male partner. The findings that were presented at the 81st ASRM Scientific Congress & Expo, provide critical data to support fertility care for transgender patients. 

While prior research on fertility care for transgender men has largely focused on egg retrieval outcomes, this study is among the first and largest to examine both embryo development and transfer outcomes in this population. 

“Our findings demonstrate that transgender men can achieve pregnancy outcomes on par with cisgender women, even in patients with prior testosterone exposure,” shares Kyle N. Le, M.D., lead author and Reproductive Endocrinology and Infertility Fellow at Shady Grove Fertility in Virginia. “This data provides important reassurance to patients and informs clinical counseling and decision-making.” 

The study analyzed 24 transgender men who underwent 27 IVF cycles and 50 cisgender women who underwent 61 IVF cycles as a comparison group. Key findings include: 

  • Cumulative live birth was 83% for embryos from transgender males and 69% for embryos from cisgender females 

  • Live birth per embryo transfer was 63% for embryos from transgender males versus 41% for cisgender females. 

  • Embryo development outcomes were comparable between groups. 

  • Among transgender males with prior testosterone exposure, live birth outcomes were similar to those without prior exposure.
